First Visit
Your initial appointment will consist of a consultation explaining your diagnosis and treatment options. Surgery can be performed the same day as the consultation when indicated. However, a review of your medical history and treatment plan may require a second appointment to provide treatment.
Please assist us by providing the following information at the time of your first appointment:
- Photo ID/Dental Insurance card is applicable
- Referral card and any x-rays if applicable.
- An up to date list of current medications
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or legal guardian at all visits.
